- Download and complete the Consumer Dispute & Consumer Copy Request form, located here. View the image below for an example of the form you will be completing.
- Contact the provider of the report, All Clear Screening, to express your interest in disputing the results. Phone and email contacts for All Clear Screening are listed here.
- All Clear Screening will notify the company of your decision and immediately begin further research of the items included in the dispute.
- After the research is completed, both you and the company are notified of the final results and provided a final report.
- The company will continue its process for deciding employment and notify you of any next steps or outcomes.
